Appendices to the Regulation
Supporting documents, forms and templates used in the implementation of the Regulation of the NASU FAIR Data Competence Center.
The appendices translate the organisational provisions of the Regulation into practical instruments for planning, monitoring, documenting and reporting the activities of the Competence Center.

Activity Indicators
A structured set of indicators used to document and assess the consultation, training, methodological, coordination and research data support activities of the Competence Center.
Operational form
Monitoring Form
A standard form for the periodic collection of information about activities, services, users, consultations, training events, partnerships and achieved results.
Reporting template
Annual Report Template
A standard structure for presenting the annual activities, services, outputs, indicators, partnerships and development priorities of the Competence Center.

How the appendices are used
The appendices provide practical instruments for implementing the organisational, monitoring and reporting provisions of the Regulation.
They may be used by the management and staff of the Competence Center, members of its coordination bodies, data experts and other authorised participants involved in planning, documenting or evaluating its activities.
The appendices should be applied together with the current approved version of the Regulation and applicable institutional acts of Kyiv Academic University.
Role of the supporting documents
Each document performs a specific function within the management and accountability framework of the Competence Center.
Regulation
Defines the mandate, functions, governance and organisational framework.
Indicators
Define what aspects of the Center’s activity should be measured.
Monitoring form
Provides a consistent mechanism for collecting operational information.
Annual report
Consolidates collected information and presents the results for the year.
Document access and version control
Reference copies of the appendices are published in PDF format. Documents intended for completion or adaptation may also be provided in editable DOCX or XLSX formats.
The status, version number and date of the latest update should be indicated on the individual page of each document. When an appendix is revised, the current version replaces the previous public version.
Where an appendix has been formally approved together with the Regulation, changes to that appendix must follow the applicable institutional approval procedure.
Users should verify the document version before using a form or template for official institutional reporting.
